Multi-agency working
The national analysis of Safeguarding Adults Reviews has found that the need for stronger communication, case coordination and multiagency risk management is a common theme in cases subject to review.
Multi-agency discussions should be considered a standard response when concerns are raised. To support effective multi-agency working, the H&F SAB has developed a template to guide multi-agency responses and support professionals in establishing shared responsibility and clear action plans.
Occasionally professionals may need to escalate concerns to established multi-agency panels within Hammersmith & Fulham. These panels have been established to support multi-agency responses where a person remains at high-risk despite previous interventions.
